IMPORTANT: Anyone that did not come to us last year for their tax preparation has to be inputted as a new client.  Please read ALL the following information, so all appropriate forms are completed.  

 

Individual Clients - Income & Gift Taxes (New Client)

 

**Single Member LLC's and Sole Proprietor file on an Individual Tax Return; therefore, you will use this page.**

IRS requires that we get certain information from each client. 

FILING FOR YOURSELF AND/OR FILING JOINTLY WITH SPOUSE:

In order to prepare your tax return, we need certain items.  The following is a list of the items we need: 

Client Intake Form (please fill this out even if there are no changes)

•We have to have a valid unexpired unaltered government-issued picture ID for the taxpayer and, if married, their spouse. 

• We need Social Security Cards for the taxpayer and, if married, their spouse.  If you or your spouse receive Social Security and received Form 1099 from Social Security then we can use that form in place of a Social Security Card.  

• If there are any dependents being claimed on your tax return, then we need each of their social security cards (if they receive Social Security and received a Form 1099 from Social Security then we will use that statement in place of a Social Security Card)

• We need, if possible, a complete copy of last year's Federal and State Tax Return.  In certain circumstances, it is required. 

• A signed Engagement Letter and Fee Agreement 

Individual Client Questionnaire 

Consent to Release Information Form (unless we have a Forever Release Form on file) - This form is required for us to respond via email or fax.  Additionally, we need a new consent form for each third party you would like information released to.  

• Any and all 1099's Forms, W2 Forms, Form 1098 Mortgage Statements, and any form that says "tax document."

• If you are a minister, you will need to complete the Minister's Deduction Worksheet 

• If you have a business, farm, or rental property please see below for additional forms.

FILING ON BEHALF OF SOMEONE ELSE:

In order to file a tax return on behalf of someone else, we need certain items.  The following is a list of the items we need: 

Client Intake Form (Complete this form, if the person is NOT deceased)

Infomation Sheet for a Deceased Individual (Only complete this is they are deceased)

• A signed Engagement Letter and Fee Agreement

Individual Client Questionnaire

• Your POA Form (Power of Attorney)

• If deceased, we need your Court Qualification Letter or Small Estate Affidavit 

• If deceased, we need a copy of the Will, if they had one.  

• If deceased, we need an original Death Certificate (we will return the original Death Certificate once we scan it)

FILING A BUSINESS, FARM, OR RENTAL PROPERTY: (Business means Single-Member LLC's or Sole Proprietor) 

These forms are in addition to the above forms.  The following is a list of the items we need: 

• If you have a farm, we need a Profit and Loss for the Filing year or use this form.  

• If you have a rental property, we need this form filled out for each rental property.  (You can use an equivalent of the form; however, there is additional information on our form that is very helpful for us when filing a tax return. 

• If you have a business, we need a Profit and Loss or you can click this link and choose the template that is most appropriate for your business. 

• For all businesses, farms, and rentals we need a break down of equipment, assets, furniture, vehicles, and improvements done in the filing year and broken down by date, cost, and description.  

• If you filed your business, farm, or rental on your tax return in a previous year, we need the Depreciation Worksheet from that tax return.

• For all businesses, farms, and rentals, if you filed 1099 Forms, we need your 1096 Summary Form. 

• For all businesses, farms, and rentals, if you had employees in the filing year, we need a copy of your W-2's and the W-3       Summary Form. 

Email: If you would like to e-mail us your information, we only take PDF format files that have been originally scanned and saved as PDFs.  You cannot convert to a PDF from a picture file.  If you have a smartphone they often times take a picture then convert it to a PDF.  When it converts it to a PDF from a picture our computers reads it as an altered document and our systems will block it.  If at all possible, please scan them into a computer and save them as a PDF to your desktop.  Then attach that PDF to your email.
If you send more than 5 PDF attachments, including in multiple e-mails, we have to put your documents in a holding bin until someone can merge them.  It will speed up the processing of your return if you merge them into 5 or fewer documents before sending them to us.  
 
How to Merge Files: Go to insert file on your PDF viewer and select it, it will ask you what you want to insert  Select the various pages you want to insert and insert them into the file you have open.  Just hit save once you are completed or you will lose everything you inserted.
